Job Description:

Under the supervision of the Program Director, Lead Linkage Specialist will provide supervision to the linkage team in assisting high risk individuals who are not engaged in care by identifying, engaging and monitoring HIV, HCV, and/or STD diagnosed persons who are newly diagnosed or have fallen out of the continuum of care and providing linkage into primary healthcare. He/she will provide case management to those in need and follow the client for a period of 18 months to ensure retention in care. Lead Linkage Specialist along with his/her team will help in increasing the self-efficacy of persons living with HIV,HCV, and/or STD in navigating the healthcare system to ensure access to timely and appropriate care and improve health outcomes. Lead Linkage Specialist along with his/her team will transport HIV, HCV, and STD positive patients to their first medical appointments and provide instruction for subsequent appointments. They will assist persons living with HIV and/or HCV in acquiring essential and support services (e.g. birth certificate, housing) that will help increase in medical and medication adherence. Furthermore they will identify and link individuals in need of housing and Substance Use treatment. Lead Linkage Specialist will furthermore be responsible in managing the counseling center and providing individual and group counseling as well as linkage to mental health and substance use treatment to the agency’s target population. Some travel is required.

Minimum Requirements:

  • Knowledge/Skills
  • Identify and recruit new and previously diagnosed HIV, HCV, and/or STI-positive individuals who have fallen out of care or not engaged in primary care and provide patient retention navigation services by utilizing a strengths-based intervention to link them back into care
  • Engage persons of unknown HIV,HCV, and STI status and connect to HIV and/or VH counseling and testing services
  • Maintain a caseload of clients enrolled in a strength-based case management intervention
  • Assist in the coordination of healthcare services and provider referrals through linkages to primary HIV,HCV, and/or STI healthcare and assist with retention in care by linkage to supportive services
  • Screen, assess and evaluate patients for readiness to re-enter and actively participate in consistent HIV, HCV, and/or STI care
  • Chart patient retention navigation progress and reporting through data entry; maintain client files according to program protocols.
  • Maintain knowledge about the latest treatment
  • Assist HIV and/or HCV positive and high-risk negative individuals in obtaining essential and support services (e.g. housing)
  • Deliver individual and group counseling
  • Ability to supervise a team

  • Excellent skills in public speaking
  • Ability to work independently
  • Ability to collect data and provide comprehensive reporting to funders and management
  • Experience/Education:
  • Masters of social work or related field
  • 2 years of qualified experience; or an equivalent combination of training, education and experience that demonstrates the ability to perform the duties of the position
  • Experience related to HIV, HCV, and/or STI policies and programs training
  • Experience working with medical providers in linking patients to care
  • Experience in navigating insurance and/or patient assistance programs
  • Experience and certificate on HIV and HCV testing and counseling is a plus
